Forms and Policies

Please arrive ahead of time to fill out medical information sheet. Please have insurance information available and current, so as not to delay your appointment. We strive hard to be on time but due to emergencies a slight delay may occur. Please make sure your referrals are current.

Payments:

Payments including co-payments and deductibles are expected at time of service. Payment is accepted in cash, check, or credit card.

Insurance:

Participants in Worker’s Compensation, no-fault, Blue Cross Blue Shield of Rochester, Medicare, blue hospice, Empire, GHI, preferred care, Monroe Plan, ValuMed. Medicaid patients are excepted from primary care referrals.

Prescriptions:

Prescriptions are preferred to be written by the doctor at the time of your office visit. If refills are needed, please plan ahead period prescriptions are not refilled on weekends or after office hours. Prescriptions are not written if the patient has not been seen recently or has a new problem without being seen and evaluated.

Emergencies:

A when emergencies arise after hours go to the nearest emergency room affiliated with this practice. The doctor on call can be contacted by the ER physician. For after hours emergency is the on-call doctor can be contacted by calling the office number which is forwarded to the answering service.

Cancellations:

Cancellations would be appreciated as soon as possible at least 24 hours in advance. This appointment can therefore accommodate other patients. Your appointment can be rescheduled. We do not appreciate “no-shows” and will not reschedule in the future.

Requests for x-rays:

Requests of x-rays required time. Please let us know several days ahead. A these will be printed on a disk. There is a charge of $15 for copying a disk and $10 for mailing.

Disability Forms and Release of Medical Records:

Disability forms take time and are handled within days. Disability forms are not filled out the same day. A charge of $5.00 per form is charged after two forms. Payment is required in advance. Your portion and your employers portion have to be completed prior to giving the form to us. Requests for records are released only by a sign release forms requested by the patient. Copies to an attorney must be requested by the attorney with signed authorization for release of records by the page requests for release of records can only be requested by the patient. A there will be a minimal charge for Xeroxing and handling. Release of records will require several days.

HIPPA:

We follow HIPPA regulations. Please do not ask us to violate these.

Meet The Doctors

Dr. Daniel Alexander

Born and raised in Buffalo a former lieutenant in the Buffalo Fire Department, Dr. Alexander received his medical degree from State University of New York at Buffalo. His orthopedic training was completed at Henry Ford Hospital in Detroit, Michigan. During his orthopedic training Dr. Alexander specialized in orthopedic sports surgery including arthroscopy of the shoulder, knee, hip, and ankle; also rotator cuff repair, ACL reconstruction, joint replacement and orthopedic trauma. He has continued to specialize in these areas in the Finger Lakes region.  He is providing both emergency care and elective surgery at these centers. He also covers the Penn Yan Soldiers and Sailors emergency room. He specializes in arthroscopic surgery of the shoulder, knee and ankle as well as total joint replacements utilizing MIS (minimally invasive surgery). He also takes care of many work related injuries including carpal tunnel surgery. Dr. Alexander would be happy to assist you with your orthopedic needs. Dr. Alexander and family resides locally.

Dr. David Cywinski

Dr. Cywinski raised in Buffalo, NY has an interesting background which includes 14 years as Fayetteville, NY firefighter and a paramedical instructor at SUNY Health Science Center. His BA degree in Biology was obtained at SUNY College at Buffalo. He received his medical degree at SUNY Health Science Center in Syracuse in 2000. He then took a dual residency in Internal Medicine and Pediatrics at the University of Rochester/Strong Memorial Hospital. He is board eligible in both. Dr. Cywinski has joined Finger Lakes Bone and Joint Center, LLP to practice non surgical orthopedic medicine.His interests are hunting, fishing and the outdoors. He also was a National Rugby Player and will practice sports medicine. He would be happy to take care of your orthopedic or sports problems and injuries. Dr. Cywinski lives locally with his family.

Scott Matoon – PA

After graduating from Midlakes High School, Scott enlisted in the United States Army, where he functioned as a combat medic for over three years. After serving as a medical specialist in the 28th Combat Support Hospital, Scott was honorably discharged to further pursue his medical education. He attended Oswego State University, where he graduated magna cum laude with a degree in biology. Scott then completed his Masters of Science degree in Physician Assistant studies at the Arizona School of Health Sciences in Phoenix, Arizona.  During his tenure at the Arizona School of Health Sciences, Scott completed rotations in OBGYN, general surgery, pediatrics, orthopedic surgery, emergency medicine, sports medicine, family practice and internal medicine. Scott is also certified in advanced cardiac life support, basic life support, and nutrition counseling. Scott has been practicing as a Physicians Assistant in orthopedics for the last five years.

Dr. Robert Supinski – Orthopedic Surgeon

Dr. Robert Supinski joined Finger Lakes Bone and Joint Center in 2009 and practices Hand Surgery and Orthopedic Surgery. Dr. Supinski is from Dansville, and graduated with an MD 30 years ago. Dr. Robert Supinski graduated from Medical School at University of Pittsburg School of Medicine, Pittsburg, PA, in 1980.  He completed his residency in Orthopedics at Pennsylvania State University Hospital, Hershey Medical Center, Hershey, PA in 1985. Dr. Supinski is Board Certified in Orthopedics and Hand Surgery.He practices in Dansville, NY and is a member of the Active Medical Staff at Noyes Memorial Hospital.
